• Complete the online fillable Deerfield Future Resident Wait List Application.
• Please note: This secure form takes approximately 30 min. to complete and requests extensive medical and financial information. For security reasons, inactivity on the application will result in the form timing out, and inputted information will be lost. We recommend opening the application, reviewing all information required, and having that information ready in advance of completing the application.
2) Apply for the Expansion Wait List.
This is a non-binding reservation agreement for those seeking residency in the new Canterbury Homes or Johnson Hall Apartments.
3) Submit the application fee.
This payment accompanies both the application for future expansion and the application for current residences.
